2021 MFL Winter Tournament – Player Registration Form

I am a member of the above named team and, as a participant, will abide by all the rules, regulations and policies set forth by the MFL Foundation.  I understand participation in athletic activities may have an element of hazard or inherent danger, and I take full responsibility for my actions and physical condition.  I hereby for myself, my heirs, executors and administrators agree to indemnify and hold the MFL Foundation, Athletico Center, it's officers, employees, volunteers and agents,  harmless from any liability, loss, cost or expense (including attorney’s fees, medical and ambulance costs) that may occur while participating.  In case of emergency, I give my permission for emergency medical treatment.  I hereby grant the MFL permission to use my likeness in photographs, videos, or other digital media in any and all of its publications, without payment or other consideration.  I understand and agree that all photos will become property of the MFL.  I waive any right to inspect or approve the finished product where my likeness appears.  Additionally, I waive the right to royalties or other compensation arising or related to the use of the photos and/or videos.  This form shall be considered valid until cancelled or changed in writing by the undersigned participant/parent/guardian prior to the event.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The coronavirus (“COVID-19”) and its variants has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. The MFL has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you will not become infected with COVID-19 during participation in the MFL Tournament.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while participating in the MFL Tournament and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at this event may result from the actions, omissions, or negligence of myself and others, including, but not limited to, the MFL coordinators, sponsors, vendors, volunteers, referees, participants and their supporters.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the MFL Tournament. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless the MFL, its coordinators, volunteers and sponsors of and from claims, including all liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the MFL, its coordinators, participants, sponsors and vendors, whether a COVID-19 infection occurs before, during, or after attending the MFL Tournament.
